In my case i was able to reproduce this bug mostly with skype capturing video and audio over usb on webcam. E3500 can't work at all with pulseaudio without this patch. The Notebook Pro cam can work but some times it get unstable.

Ignoring ctrl errors seems to fix both cases.

Thanks. Maybe digging deeper to look for the real culprit would be better, but your bandaid looks simple enough, so I applied the patches now.
